Title: Staff Pharmacist - Pharmacy
Location: Southern Colorado
Shift : Day
FT (32 hrs/wk + call); days and weekends.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Reviews physician medication orders for accuracy; appropriate dose, frequency, potential side effects, appropriate administration, and indications.
2. Provides all acute care, procedural, emergency, and standard medications for patients of the hospital.
3. Provides IV admixture services utilizing aseptic technique and ensuring medication stability and compatibility.
4. Ensures maintenance of efficient and current inventory of formulary approved emergency, acute care, biological, and IV medications.
5. Performs appropriate medication order computer entry and patient profile maintenance.
a. Screens appropriateness of all medication doses.
b. Screens all medication doses versus patient renal function.
c. Screens patient profiles for drug interactions.
d. Screens patient drug allergies versus ordered medications.
e. Ensures scheduling of medication to minimize patient adverse reactions and maximize efficacy.
f. Monitoring of patient adverse drug reaction events.
Compensation: $95,000 to $110.000
EDUCATION and/or EXPERIENCE
Bachelor of Science or Doctor of Pharmacy graduate from an accredited school of pharmacy with two postgraduate years of hospital pharmacy practice
experience.
CERTIFICATES, LICENSES, REGISTRATIONS
Current Colorado State Board of Pharmacy Licensure.
