Degree Type/Area: BSW or related field required. Master’s degree preferred

Description:

Case Managers provide an array of services to children and families that represent best practices. This position focuses on the prevention of inappropriate out-of-home placement, strengthening and improving family functioning, and ensuring child safety.

Specific Responsibilities:

  • Provide in-home services to families and children based on identified needs.
  • Screen new referral within 24 hours.
  • Develop strength-based assessment and service plan.
  • 24/7 pager coverage.
  • Teach basic coping and life skills such as budgeting, parenting, communicating, problem solving, etc.
  • Link families to appropriate community resources.
  • Assess child and family safety and risk.
  • Develop individualized case plans.
  • Oversee case responsibilities.
  • Assists families with identifying and removing barriers that prevent children from remaining or being returned home.
  • Conduct home visits based on individual family need.
  • Maintain accurate home visit notes.
  • Refer families to community resources and provide linkages with necessary treatment providers.
  • Staff all cases with case manager supervisor weekly.
  • Complete required written documents in a timely manner.
  • Work with families to ensure permanency for clients.
  • Complete various outcome reports as required.
  • Attend court hearings, case reviews, staffing and meetings.
  • Ensure required documents are in case files.
  • Ensure unusual incidents reports are completed accurately and promptly.

  • Ability to work with resistant families.
  • Ability to use confrontation when necessary to produce change.
  • Ability to teach basic coping and problem-solving skills.
  • Knowledge of and ability to teach parenting skills.
