~ One (1) Part Time Position ~

The Physical Therapist is responsible for modalities that promote physical comfort, safety and maintenance of optimum independence for the patient in the home setting.

The Physical Therapist is responsible for the management of all care in relation to therapy needs. The Physical Therapist supports and fosters the philosophy, goals and objectives of the agency. Reports to the Supervisor Clinical Services.

Essential Responsibilities:
1. Provides skilled Physical Therapy services to patients and families. 75%
2. Develops the patient care plan in conjunction with other members of the team. 20.0%
3. Participates in agency committee meetings and performs other agency related duties. 2.5%
4. Participates in continuing education and professional development. 2.50%
5. Adheres to General Performance Standards.


WORKING ENVIRONMENT: Home visits to patients. Sitting, standing, walking, bending, lifting. Potential exposure to blood and body fluids. High stress environment related to dealing with patient illness.

Normal heat, light and air in office facility. High telephone usage. Frequent interruptions.

Physical and Mental Effort: When providing patient care, this position requires exerting up to 60 pounds of force with bending, kneeling, reaching, pushing, and pulling movements routinely. When not providing patient care, this position involves sitting most of the time, with walking or standing for brief periods, exerting up to 10% of force occasionally and negligible amounts of force frequently.

This position involves daily driving an automobile varied distances to client homes and the ability to enter/exit an automobile numerous times daily. The Physical Therapist must also have the ability to enter buildings using varied levels of stairs in order to provide service to patients.

The Physical Therapist must possess good written and oral communication skills and be able to work in a variety of environments. In addition, the Physical Therapist must be able to work cooperatively with members of the Interdisciplinary Team to support home care patients and families.

Comprehensive knowledge of the principles, concepts and methodologies of Physical Therapy and the practical application in the treatment of patients in the home.

Graduate of an accredited school of Physical Therapy
BS Degree in Physical Therapy Master's Degree preferred
Connecticut License to practice Physical Therapy Current
