Citizens for Quality Sickle Cell Care, Inc. (CQSCC), is a nonprofit agency that provides on-going education about Sickle Cell Disease for health care providers, families, employers, educators, and the community. Our organization is made up of volunteers from Connecticut.. CQSCC emphasizes the need to promote quality and up-to-date comprehensive medical care along with support and involvement in advocacy for on-going life span transitional programs and support services. CQSCC is currently seeking a Case Manager to work with members, individuals and families who are living with Sickle Cell Disease in New Britain.
Duties include:
· Complete intake tasks for new referrals
· Education and referral assistance to our members so that they can receive support and assistance from other appropriating agencies
· Connection go medical homes for our members, including linkage to primary care physicians
· Linkage to support group opportunities
· Other duties as required
Qualifications:
· Knowledge and background in case management
· Ability to work with people from varying socio-economic levels
· Ability to multi task with experience in developing and fulfilling independent projects
· Basic networking experiences in the Greater New Britain area
· Basic knowledge about Sickle Cell Disease
· General technology/computer knowledge
